Note. Fees/Donations are not required to participate. However, for this form to send, you must enter an option for Donation Box, even if you are unable to make any donation. If you can not make a donation you must enter "Other" or the form will not Send. If you can donate, donations methods are by money or by a small size sculpture or by both.
